Меры предосторожности:
Лечение в сочетании кортикостероидов и антибиотиков не следует продолжать лечение более 7 дней, даже при отсутствии какого-либо клинического улучшения, так как длительное применение может привести к распространению инфекций.
Длительное применение может привести к риску угнетения надпочечников у младенцев.
Не рекомендуется детям младшего возраста или младенцам.

Безопасность применения при беременности и кормлении грудью не установлена.

Свойства:
Фрамицетин является аминогликозидным антибиотиком широкого спектра действия для местного применения. Воздействие фрамицетина нарушает синтез белков, жизненно важных для функционирования бактериальной клетки, что приводит к гибели микроорганизма. ⠀
Дексаметазон за счет противовоспалительного, противоаллергического и десенсибилизирующего действия уменьшает боль, зуд, жжение в наружном слуховом проходе, устраняет ощущение заложенности уха.
Цинхокаин анестетик, снимает болевые ощущения. Механизм действия обусловлен блокадой чувствительных нервных окончаний в коже или слизистых оболочках, что связано со стабилизацией мембран нейронов и предотвращением возникновения и проведения нервного импульса.

2-3 капли закапывают в каждое ухо три-четыре раза в день.
Продолжительность лечения не более 7 дней
Противопоказания:

Otal® ear drops – Anti-inflammatory and broad spectrum antibacterial agent with topical anesthetic
Composition
Each 100 ml contains:
- Framycetin sulphate: 390mg Equivalent to 300 mg Framycetin base
- Gramicidin: 2.5mg
- Dexamethasone: 100mg
- Cinchocaine HCl: 1 gm
Pharmacological properties
Framycetin sulphate: A fermentation product of streptomyces fradii, framycetin is a bactericidal for many gram + ve and gram -ve organisms. It is an aminoglycoside antibiotic which inhibits protein synthesis by binding to ribosomal RNA, causing misreading of the bacterial genetic code.
Gramicidin: Is a bactericidal for a variety of gram + ve organisms. It increases the permeability of the bacterial cell membrane to inorganic cations by forming a network of channels through the normal lipid bi-layer of the membrane.
Dexamethasone: Is a synthetic flourinated coriticosteroid that suppresses the inflammatory response, with an additional anti-edematous and anti-pruritic effects.
Cinchocaine: Topical anesthetic of the amide type that is used for the surface anesthesia.
Indications
- For the treatment of superficial bacterial infections of the external auditory canal, caused by organisms sensitive to the antibiotics in Otal® ear drops
- In cases of severe inflammatory conditions of the external ear including edema, pruritus and other dermal reactions of the external auditory meatus.
- For the relief of pain in cases of otalgia.
Otal® ear drops is recommended for usage in cases of:
- Acute diffuse otitis externa.
- Furunculosis of the external auditory canal.
- Acute congestive or serous otitis media with an intact ear drum.
Adverse effects
As with any topical therapy hypersensitivity reactions including contact dermatitis, burning, erythema, rash and urticaria could occur in some cases, if so discontinuation of therapy and consultation of physician is recommended.
Precautions
Topical corticosteroid, when used in combination with framycetin, may mask the clinical signs of fungal and viral infections.
Otal® ear drops is not recommended in cases of perforated tympanic membrane.
Contraindications
- This product is contraindicated in those individuals who have shown hypersensitivity to any of its components and in cases of viral or fungal infections.
- Not to be used for the eyes
Dosage
The external auditory canal should be cleansed and dried with a sterile cotton applicator
- Adults: 3-5 drops of Otal® should be instilled into the affected ear(s) 3-4 times daily
- Infants and children below 3 years of age: 2-3 drops Otal® are suggested because of the smaller capacity of the ear canal.
- Alternatively: A cotton wick or gauze saturated with the solution is to be inserted into the external auditory canal and should be kept moist by adding further solution every 4 hrs. The wick should be replaced every 24 hrs.

ACTIVE-INGREDIENT OF OTAL DROPS

DOSAGE OF OTAL DROPS
Follow all directions given to you by your pharmacist or doctor carefully . They may differ from the information contained in this leaflet

The recommended dose is : –
adults (and the elderly ) and children : two or three drops instilled into the ear three or four times daily . Treatment duration should be short ( not to exceed 7 days ) .
OVER-DOSAGE OF OTAL DROPS
long-term intensive topical use may lead to systemic effects . Oral ingestion of the contents of one bottle (up to 10 ml ) is unlikely to lead to any serious adverse effects .
CONTRA-INDICATION OF OTAL DROPS
otitis externa should not be treated when the eardrum is perforated because of the risk of ototoxicity . Hypersensitivity to framycetin sulphate , dexamethasone , gramicidin or to any of the excipients .
STORAGE OF OTAL DROPS
Store in temperature not exceeding 30 °C .
