The prevention and treatment of calcium deficiency/vitamin D deficiency especially in the housebound and institutionalised elderly subjects. Absolute contra-indications are hypercalcaemia resulting for example from myeloma, bone metastases or other malignant bone disease, sarcoidosis; primary hyperparathyroidism and vitamin D overdosage. Severe renal failure. Hypersensitivity to any of the tablet excipients.
Relative contra-indications are osteoporosis due to prolonged immobilisation, renal stones, severe hypercalciuria.
Adcal-D3 Dissolve contains a small quantity of soya oil and is therefore contraindicated in patients who are allergic to peanuts or soya.

Patients with mild to moderate renal failure or mild hypercalciuria should be supervised carefully including periodic checks of plasma calcium levels and urinary calcium excretion.
In patients with a history of renal stones urinary calcium excretion should be measured to exclude hypercalciuria.
With long-term treatment it is advisable to monitor serum and urinary calcium levels and kidney function, and reduce or stop treatment temporarily if urinary calcium exceeds 7.5mmol/24 hours (300mg/24 hours).

Adcal-D3 Dissolve should also be used with caution in other patients with increased risk of hypercalcaemia e.g. patients with sarcoidosis or those suffering from malignancies.
Patients with rare hereditary problems of fructose intolerance, glucose-galactose malabsorption or sucrase-isomaltase insuffiency should not take this medicine.
Each tablet contains a small amount of sugar (about 1.7 mg per tablet) and may be harmful to teeth if used for a prolonged period.
Allowances should be made for calcium and vitamin D supplements from other sources.

Hypersensitivity reactions including pruritus, wheezing, urticaria and oropharyngeal swelling have been reported in the postmarketing environment.
The use of calcium supplements has, rarely, given rise to mild gastro-intestinal disturbances, such as constipation, flatulence, nausea, gastric pain, diarrhoea. Following administration of vitamin D supplements occasional skin rash has been reported. Hypercalciuria, and in rare cases, hypercalcaemia have been seen with long term treatment at high dosages.

The most serious consequence of acute or chronic overdose is hypercalcaemia due to vitamin D toxicity. Symptoms may include nausea, vomiting, polyuria, anorexia, weakness, apathy, thirst and constipation. Chronic overdoses can lead to vascular and organ calcification as a result of hypercalcaemia. Treatment should consist of stopping all intake of calcium and vitamin D and rehydration.

Pharmacotherapeutic group: A12AX01 Calcium carbonate and colecalciferol
Strong evidence that supplemental calcium and vitamin D3 can reduce the incidence of hip and other non-vertebral fractures derives from an 18 month randomised placebo controlled study in 3270 healthy elderly women living in nursing homes or apartments for elderly people. A positive effect on bone mineral density was also observed.
In patients treated with 1200mg elemental calcium and 800IU vitamin D3 daily, i.e. the same dose delivered by two tablets of Adcal-D3 Dissolve, the number of hip fractures was 43% lower (p=0.043) and the total number of non vertebral fractures was 32% lower than among those who received placebo. Proximal femur bone mineral density after 18 months of treatment increased 2.7% in the calcium/vitamin D3 group and decreased 4.6% in the placebo group (p < 0.001). In the calcium/vitamin D3 group, the mean serum PTH concentration decreased by 44% from baseline at 18 months and serum 25-hydroxy-vitamin D concentration had increased by 162% over baseline.
Analysis of the intention-to-treat results showed a decreased probability of both hip fractures (p = 0.004) and other fractures (p < 0.001) in the calcium/vitamin D3 treatment group. Analysis of the other two populations (active treatment and those treated and followed for 18 months) revealed comparable results to the intention-to-treat analysis. The odds ratio for hip fractures among women in the placebo group compared with those in the calcium/vitamin D3 group was 1.7 (95% CI 1.0 to 2.8) and that for other nonvertebral fractures was 1.4 (95% CI 1.4 to 2.1). In the placebo group, there was a marked increase in the incidence of hip fractures over time whereas the incidence in the calcium/vitamin D3 group was stable. Thus treatment reduced the age-related risk of fracture at 18 months (p = 0.007 for hip fractures and p = 0.009 for all non-vertebral fractures). At 3 years follow-up, the decrease in fracture risk was maintained in the calcium/vitamin D3 group.

The pharmacokinetic profiles of calcium and its salts are well known. Calcium carbonate is converted to calcium chloride by gastric acid. Calcium is absorbed to the extent of about 15-25% from the gastro-intestinal tract while the remainder reverts to insoluble calcium carbonate and calcium stearate, and is excreted in the faeces.
The pharmacokinetics of vitamin D is also well known. Vitamin D is well absorbed from the gastro-intestinal tract in the presence of bile. It is hydroxylated in the liver to form 25-hydroxycolecalciferol and then undergoes further hydroxylation in the kidney to form the active metabolite 1, 25 dihydroxycolecalciferol (calcitriol). The metabolites circulate in the blood bound to a specific α-globin. Vitamin D and its metabolites are excreted mainly in the bile and faeces.

Описание
Таблетки светло-оранжевого цвета с вкраплениями, закругленные, продолговатые с двояковыпуклой поверхностью, с риской с одной стороны и тиснением «ALB400» с другой стороны, с характерным фруктовым запахом.

Фармакологические свойства
Фармакокинетика
После перорального приема албендазол плохо адсорбируется из ЖКТ
(меньше 5%), в неизмененном виде не определяется в плазме крови, биодоступность — низкая.
Системное фармакологическое действие увеличивается, если доза принимается с жирной пищей, которая повышает всасывание и максимальную концентрацию в плазме (Сmax) в 5 раз. Время достижения Сmax албендазола сульфоксида составляет 2-5 ч. Связывание с белками плазмы -70%, проникает в значительных количествах в желчь, печень, спиномозговую жидкость, мочу, стенку и жидкости цист гельминтов. Метаболизируется в печени с образованием первичного метаболита альбендазола сульфоксид, обладающего антигельминтной активностью. Албендазола сульфоксид метаболизируется в албендазола сульфон (вторичный метаболит) и другие окисленные продукты.
После приема разовой дозы 400 мг фармакологически активный метаболит, сульфоксид албендазола, достигает плазменных концентраций от 1,6 до 6,0 микромоль/л при приеме препарата с пищей. Период полувыведения албендазола сульфоксида составляет 8,5 ч. Выводится с желчью через кишечник в виде албендазола сульфоксида, и лишь незначительная его часть выводится с мочой.
При поражении печени биодоступность повышается, при этом Сmax альбендазола сульфоксида увеличивается в 2 раза, а период полувыведения удлиняется. Индуцирует цитохром Р450 IA в клетках печени человека.
Фармакодинамика
Зентел – противопротозойный и антигельминтный препарат широкого спектра действия, производное бензимидазола карбамата.
Зентел нарушает процессы транспорта глюкозы, вызывает расстройство функции микротубулярного аппарата в организме гельминта, что приводит к его гибели и выведению из организма человека.

Способ применения и дозы
Препарат принимают внутрь вместе с приемом пищи. Предпочтителен прием в одно и то же время суток.
У некоторых пациентов могут возникнуть трудности с проглатыванием таблетки; в таких случаях таблетку можно разжевать и запить небольшим количеством воды или растолочь.
Если не наступает выздоровления через три недели, прописывается второй курс лечения.

Пожилые пациенты
Опыт применения у пациентов старше 65 лет ограничен. Корректировки дозы не требуется, однако следует соблюдать осторожность при назначении пациентам с нарушениями функции печени.

Так как выведение албендазода и его первичного метаболита, албендазол сульфоксида, незначительно, влияние почечной патологии на почечный клиренс маловероятно. Однако, у пациентов с почечной недостаточностью следует проводить мониторинг функции почек при назначении Зентела.
Печеночная недостаточность
В связи с тем, что албендазол быстро метаболизируется посредством печеночного метаболизма до первичного метаболита албендазол сульфоксида, нарушение функции печени оказывает значительное влияние на фармакокинетику албендазол сульфоксида. Пациенты с нарушением показателей печеночных тестов (трансаминз) должны бать тщательно обследованы до момента назначения препарата Зентел.
Системные гельминтные инфекции
(продолжительное лечение более высокими дозами).
Режим дозирования устанавливается индивидуально в зависимости от возраста, массы тела, а также от степени тяжести инфекции.
Доза для пациентов при массе тела больше 60 кг составляет 400 мг 2 раза в сутки. При весе меньше 60 кг препарат назначается из расчета 15 мг/кг/ в сутки. Эта доза должна быть разделенная на 2 приема. Максимальная суточная доза составляет 800 мг.

Зентел назначается в случае неоперабельной патологии; до оперативного вмешательства; в пост-операционном периоде, когда курс предоперационной терапии был слишком коротким или найдены жизнеспособные кисты; после чрезкожного дренирования цист в диагностических или терапевтических целях. Курс лечения составляет 28 дней. При неоперабельной форме перитонеальных кист, кист в легких и печени рекомендуется три 28- дневных цикла лечения, разделенных 14-дневным перерывом в приеме препарата. При наличии кист в головном мозге и костях может потребоваться более длительный курс терапии. Перед операцией рекомендовано два 28-дневных цикла, разделенных 14-дневным перерывом в приеме препарата. Если хирургическое вмешательство необходимо до момента окончания двухциклового курса, Зентел должен приниматься до момента оперативного вмешательства. Если дооперационный курс был меньше 14 дней или найдены жизнеспособные кисты, то после операции проводят два цикла по 28 дней, разделенных 14-дневным перерывом в приеме препарата. |

Лекарственные взаимодействия
Циметедин, празиквантел и дексаметазон увеличивают плазменные уровни метаболита албендазола, ответственного за системную эффективность продукта.
Ритонавир, фенитоин, карбамазепин и фенобарбитал потенциально могут снижать плазменную концентрацию активного метаболита албендазола – албендазола сульфоксида. Клиническое значение данного факта неизвестно, однако может влиять на эффективность Зентела, особенно при лечении системных гельминтозов. Пациенты должны находиться под наблюдением врача на предмет эффективности лечения, что может потребовать назначения альтернативного режима дозирования или пересмотра проводимой терапии.
Особые указания
Чтобы избежать приема Зентела в течение ранней беременности, женщины детородного возраста должны проводить лечение в течение первой недели менструации или после отрицательного теста на беременность. Тест на беременность должен быть повторен как минимум один раз перед началом следующего цикла. Следует принимать меры предосторожности против зачатия во время и в пределах одного месяца после завершения приема препарата при системной инфекции.
В процессе лечения пациентов препаратом Зентел может выявится наличие у них существующего нейроцистициркоза, особенно у пациентов в зоне с высокой степенью распространения тенозных инфекций.У пациентов, лечащихся от нейроцистициркоза и системных гельминтных инфекций, могут проявляться неврологические симптомы на фоне приема Зентела, связанные с гибелью паразитов. К данным симптомам относятся припадки, повышение внутричерепного давления и очаговые симптомы. Симптомы могут проявиться сразу после начала терапии и требуют незамедлительного лечения стероидами и антикольвульсантами. Пероральные или внутривенные кортикостероиды рекомендуются к применению в начале первой недели терапии для предотвращения эпизодов внутричерепной гипертензии.
Возможно слабое или умеренное повышение уровня печеночных энзимов, которые нормализируется после прекращения приема Зентела.
В процессе терапии были выявлены случаи развития гепатита. Определение показателей функции печени должно проводиться перед началом каждого цикла лечения и каждые две недели в течение лечения. Если показатели трансаминаз увеличиваются в два раза и более, Зентел нужно отменить. Лечение возобновляют, когда показатели функции печени возвращаются к норме, но пациенты должны находиться под наблюдением лечащего врача.
Зентел может вызывать подавление функции костного мозга, в связи с чем анализы крови должны выполняться в начале и каждые две недели в течение каждого 28-дневного цикла. Пациенты с заболеваниями печени, включая печеночный эхинококкоз, имеют более выраженную тенденцию к подавлению костного мозга, ведущую к панцитопении, апластической анемии, агранулоцитозу и лейкопении, что требует тщательного мониторинга картины крови. При клинически значимом снижении уровня форменных элементов крови, Зентел следует отменить.
Особенности влияния лекарственного средства на способность управлять транспортным средством или потенциально опасными механизмами
Побочные действия, влияющие на способность вождения транспортных средств и обслуживание механического оборудования в движении, не наблюдались.

По 1 таблетке помещают в контурную ячейковую упаковку из пленки поливинилхлоридной и фольги алюминиевой.
Каждую контурную ячейковую упаковку вместе с инструкцией по применению на государственном и русском языках помещают в пачку из картона.

Zentius-D
Calcium carbonate reacts with gastric acid to produce a salt and water. For calcium carbonate the postulated chemical reaction is:
CaCO3 + 2HCl = CaCl2 + H2O + CO2
Indicated in raised calcium requirement e.g. during pregnancy and lactation, and in children and adolescents at time of rapid growth, inadequate intake of calcium in the diet due to malnutrition, prevention and treatment of osteoporosis, disorders of osteogenesis and tooth formation, latent tetany.
Gastric-peptic disease occurs as a result of an imbalance between protective factors, such as mucus, bicarbonate, and prostaglandin secretion, and aggressive factors, such as hydrochloric acid, pepsin, and Helicobacter pylori (H. pylori). Antacids work by restoring acid-base balance, attenuating the pepsin activity and increasing bicarbonate and prostaglandin secretion. The acid-neutralizing capacity of calcium carbonate is 58 mEq/15 ml.When used as a nutritional supplement, calcium carbonate acts by directly increasing calcium stores within the body.
A vitamin D that can be regarded as a reduction product of vitamin D2.
Dihydrotachysterol is hydroxylated in the liver to 25-hydroxydihydrotachysterol, which is the major circulating active form of the drug. It does not undergo further hydroxylation by the kidney and therefore is the analogue of 1, 25-dihydroxyvitamin D. Dihydrotachysterol is effective in the elevation of serum calcium by stimulating intestinal calcium absorption and mobilizing bone calcium in the absence of parathyroid hormone and of functioning renal tissue. Dihydrotachysterol also increases renal phosphate excretion.
Uses
Adult: One Calcium Carbonate 500 tablet or as directed by the physician. For the prevention of osteoporosis, 1-3 Calcium Carbonate 500 tablet is recommended generally as a dietary supplement . Doses for children is half of those for adults. A large dose should not be taken without physician\’s advice.
Adolescent: One to two Calcium Carbonate tablet daily.
Children: One Calcium Carbonate tablet daily.
Dihydrotachysterol is a synthetic analog of vitamin D that does not require renal activation like vitamin D2 or Vitamin D3.
Used for the prevention and treatment of rickets or osteomalacia, and to manage hypocalcemia associated with hypoparathyroidism or pseudohypoparathyroidism. Also used for the treatment of vitamin D dependent rickets, rickets or osteomalacia secondary to long-term high dose anticonvulsant therapy, early renal osteodystrophy, osteoporosis (in conjunction with calcium), and hypophosphatemia associated with Fanconi syndrome (with treatment of acidosis).

How Zentius-D works
Calcium carbonate is a basic inorganic salt that acts by neutralizing hydrochloric acid in gastric secretions. It also inhibits the action of pepsin by increasing the pH and via adsorption. Cytoprotective effects may occur through increases in bicarbonate ion (HCO3—) and prostaglandins. Neutralization of hydrochloric acid results in the formation of calcium chloride, carbon dioxide and water. Approximately 90% of calcium chloride is converted to insoluble calcium salts (e.g. calcium carbonate and calcium phosphate).
Once hydroxylated to 25-hydroxydihydrotachysterol, the modified drug binds to the vitamin D receptor. The bound form of the vitamin D receptor serves as a transcriptional regulator of bone matrix proteins, inducing the expression of osteocalcin and suppressing synthesis of type I collagen. Vitamin D (when bound to the vitamin D receptor)stimulates the expression of a number of proteins involved in transporting calcium from the lumen of the intestine, across the epithelial cells and into blood. This stimulates intestinal calcium absorption and increases renal phosphate excretion. These are functions that are normally carried out by the parathyroid hormone.

Dosage
Zentius-D dosage
Calcium Carbonate is always used orally and when used as an antacid the recommended doses for adults are equivalent to 540-2000 mg Calcium Carbonate per day, doses for children being half of those for adults. As a dietary supplement, such as for the prevention of osteoporosis, 1250-3750 mg Calcium Carbonate (500-1500 mg calcium) daily is recommended in general, but again this will need to be tailored to the individual patient depending on any specific disease such as Calcium deficiency, malabsorption or parathyroid function. In pregnancy and lactation the recommended daily dose of calcium is 1200-1500 mg. In chronic renal failure the doses used vary from 2.5 — 9.0 gm Calcium Carbonate per day and need to be adjusted according to the individual patient. To maximize effective phosphate binding in this context the Calcium Carbonate should be given with meals.
Side Effects
In rare cases, flatulence, diarrhoea or constipation.
Toxicity
Toxicity associated with dihydrotachysterol is similar to that seen with large doses of vitamin D.
Precaution
In the presence of mild hypercalciuria, excretion levels must be carefully monitored and where necessary the dose of calcium carbonate should be reduced or treatment should be stopped. Patients with a history of stone formation should also be recommended to increase their fluid intake. High dosage of vitamin D should be avoided during Calcium therapy unless specifically indicated.
Interaction
Oral calcium can reduce internal absorption of tetracycline and fluoride prepa-rations and an interval of at least 3 hours should therefore be allowed between ingestion of these medications. Vitamin D increases internal absorption of calcium. The intestinal uptake of calcium may be reduced by concomitant ingestion of certain foods (e.g. spinach, milk and milk products).
Volume of Distribution
Calcium is rapidly distributed taken up by skeletal tissues following absorption and distribution into extracellular fluids. Bone contains 99% of the body’s calcium and the remaining 1% is approximately equally distributed between intracellular and extracellular fluids.
Elimination Route
Maximal absorption occurs at doses of 500 mg or less taken with food. Oral bioavailability depends on intestinal pH, the presence of food and dosage.
Elimination Route
Excreted mainly in the feces. The majority of renally filtered calcium is reabsorbed in the ascending limb of the loop of Henle and the proximal and distal convoluted tubules. Also secreted by sweat glands.
Pregnancy & Breastfeeding use
Pregnant women : Calcium containing drugs are used widely in pregnancy by way of calcium supplement or antacid therapy. No relationship between malformation in general and calcium exposure has been noted.
Lactating mother : There is no contraindication to the use of calcium carbonate in lactating mother.
Contraindication
Hypersensitivity to the Calcium Carbonate or any inactive ingredient of the medication. Hypercalcemia (e.g. in hyperparathyroidism, overdosage of vitamin D, demineralizing tumours such as plasmacytomas and bone metastases), severe hypercalcuria, several renal insufficiency.
Special Warning
USE IN CHILDREN: Calcium carbonate has been extensively studied in children and infants with chronic renal failure and is both safe and effective.
USE IN ELDERLY: In case of elderly patients with renal failure when calcium carbonate is taken constipation may be troublesome one for this group. For this reason, monitoring of serum calcium and phosphate is of course indicated for elderly patients.
